De acuerdo a la Resolución Ministerial 0578-2020 MTC/01, en PeruRail hemos implementado medidas de precaución para evitar la propagación del COVID-19 que permitirá un viaje con toda la seguridad necesaria , por tal motivo, es de carácter obligatorio completar la Ficha Sintomatológica 24 horas previos a su fecha de viaje y presentarla para poder abordar el servicio de trenes.
According to Ministerial Resolution 0578-2020 MTC/01, at PeruRail we have implemented new safety measures to avoid the spread of COVID-19, which will guarantee a safety journey, and for this reason, it is mandatory to complete this Symptomatology Form COVID-19 with no more than 24 hours prior to the travel date and present it correctly completed and signed to board our trains services
Declaro bajo juramento, que no presento en el momento actual signos ni síntomas respiratorios compatibles con COVID-19 y que no he estado expuesto a personas con la enfermedad o con los mismos síntomas en los últimos 14 días.

Nro. 578-2020-MTC/01, de manera que se tenga el registro de la información necesaria para realizar la trazabilidad en caso de sospecha o confirmación de contagio de la COVID-19, lo que permitirá usar, brindar y/o transferir esta información para dar cumplimiento a las obligaciones y/o requerimientos que se generen en virtud de las normas vigentes en el ordenamiento jurídico peruano, incluyendo, las que correspondan al sistema regulatorio sanitario y normas afines, por lo que el PASAJERO acepta proporcionar sus datos personales a PERURAIL, con el objetivo de ser utilizados para los fines correspondiente.
Through this consent, the PASSENGER declares to have been informed that his/her personal data of an identification nature, as well as symptomatology and exposure linked to COVID-19, will be collected and stored for the purposes described in M.R. N°. 578-2020-MTC/01, so that the information necessary to carry out traceability is recorded in case of suspicion or confirmation of COVID-19 contagion, which will allow using, providing and/or transferring this information to comply with the obligations and/or requirements arising under the rules in force in the Peruvian legal system, including those corresponding to the health regulatory system and related rules; accordingly, the PASSENGER agrees to provide his/her personal data to PERURAIL, in order to be used for the corresponding purposes
